Important:
Our team does not have access to these records or requests. All notes, reports, and imaging must be obtained through your patient portal or by submitting a Release of Information (ROI).
How to Access Your Clinic Notes
Login to your patient portal at TCOmn.FollowMyHealth.com using your email address and password. (You would have received an invitation at your initial visit to a TCO appointment.)
If you are unable to access, please email a request to [email protected] or by calling 952.512.5655 to reset your password.
How to Access Your Imaging (2-Step Process)
Step 1: Submit Release of Information (ROI)
Before imaging can be released, you must submit a signed ROI form.
What to do:
- Download and fill out all required sections of this Medical Records Request Form.
- Include:
- Patient full name
- Date of birth
- Contact information
- Specific records being requested
- Signature and date
How to submit:
- Print and sign the form
- Scan or take a clear photo
- Email as an attachment to: [email protected]
Processing time:
Allow 7–10 business days
Note: Incomplete or unsigned forms will delay processing.
Step 2: Download Your Imaging
Once your ROI has been processed, you will receive two emails to access your imaging.
Follow these steps:
- Open first email from “Medical Imaging Study – TCO”
- Open second email that includes a personal access code
- Click on the hyperlink in the first email to download your data (link valid for 14 days)
- Enter your personal code from second email
- Download images to your personal computer
- Save the files to a personal media device/USB for future appointments
